APPLICATION PROCESS and REQUIREMENTS

To register for the Department's online system, applicants need to provide their Personal Data Sheet and select the division they want to be ranked. An application number will be issued once submitted. The number must be indicated in the submission of the requirements. The head of the elementary or secondary school will receive the application.

All applicants shall register to the Department’s online system at application.deped.gov.ph.

An applicant shall submit to the head of the elementary or secondary school where a teacher shortage or vacancy (regular and/or natural) exists, a written application, with the Applicant Number indicated, supported by the following documents:

  1. Table of Contents
  2. Letter of intent (aka cover letter)
  3. Resume
  4. CSC Form 212 (aka PDS – Personal Data Sheet)
  5. PRC License – photocopy
  6. LET Rating – photocopy
  7. Transcript of Record (aka TOR) – photocopy
  8. Service Records (aka certificate of employment) – photocopy
  9. Specialized Training and Seminars Certificates – photocopy
  10. Voters ID or COMELEC Certification – photocopy
  11. NBI Clearance – photocopy
  12. Omnibus Certification of authenticity and veracity of all documents submitted

Note: Requirements may vary depending on the school to which you applied.
